Procedure for Filing An Appeal From a Common Pleas Court or Municipal Decision
File the following with the clerk of the trial court (NOT the court of appeals) within 30 days of the date of the judgment being appealed:
A Notice of Appeal. Attach a copy of the judgment entry that you are appealing.
A Praecipe and Docketing Statement Form. See Self-Representation Guide for a description of this form, instructions, and a sample copy.
A $175 filing fee. If you cannot afford to pay the filing fee, you may demonstrate that fact to the court by filing an Affidavit of Indigency or proof that you were determined to be indigent in the trial court. You may file the affidavit or other proof in place of the filing fee.
Note
Both the Ohio Rules of Appellate Procedure and the Local Rules of the Eighth District Court of Appeals will apply to your appeal and you are expected to comply with them. Both of these can be found on the court’s website. Additionally, consult the Self-Representation Guide and adhere to the warning for pro se litigants.
